Participant Mailing—Required ACA Notice
This letter and Form 1095-B will be mailed by Businessolver to all HealthFlex participants. Form 1095-B is a new, required form that verifies the “individual mandate” for health coverage under Affordable Care Act provisions.
- Participants should keep the letter and Form 1095-B with their 2015 tax documents, but are not required to file the form with their 2015 individual tax returns.
- Participants may call the General Board’s Health Team at 1-800-851-2201 with corrections to their reported data. However, this is optional—the IRS is allowing a “good faith effort” for this tax year.
Businessolver will mail the generic letter and Form 1095-B to all participants, regardless of their employer size (less than or more than 50 employees). Additionally, the letter directs questions to a human resources department, but participants may call the General Board’s Health Team at 1-800-851-2201 with corrections or questions. HealthFlex was not able to customize the letter for our population.
Requests for corrections should be sent to the Health Team by April 30; Businessolver will submit the full, corrected filing to the Internal Revenue Service in May.

Participant Mailing—Blueprint for Wellness Postcard
This postcard will be mailed this week to all HealthFlex participants. The postcard encourages them to pre-register for a screening.
