New Enrollment Form Available—Update Your Materials

Wespath recently updated the Enrollment form used by plan sponsors to enroll new participants in Wespath-administered plans. This new, streamlined form incorporates other, previously separate, forms in the Enrollment Kit, including the contribution and investment election forms. New participants need only complete this one form to be enrolled in all Wespath-administered retirement and welfare plans for which they are eligible.

Please recycle all printed copies of the old Enrollment form and begin using the new Enrollment form immediately. Stand-alone contribution and investment election forms will still be used for changes post-enrollment. We will encourage participants to update information, especially beneficiaries, online via Benefits Access after receiving their welcome letters. If your participants have already submitted their information using the old forms, there is no need to complete new forms.
