HealthFlex Enrollment/Change Form Updated
The HealthFlex Enrollment/Change Form was recently updated. Primary changes include:
- Addition of an “Election” section for plan sponsors to record medical, dental, medical reimbursement account (MRA) and dependent care account (DCA) choices for their active clergy/employees and pre-65 retirees
- Alterations to reflect the vendor’s name change from Extend Health to OneExchange

Blueprint for Wellness Postcard and Toolkit
A promotional postcard will be mailed to all HealthFlex participants during the week of March 30 to encourage them to preregister for the Blueprint for Wellness screening (April 1 – July 31). Preregistration is strongly recommended for on-site screenings during annual conference or staff wellness events and is required for screenings at a local Quest Diagnostics location. HealthFlex participants and covered spouses will each earn $100 HealthCash for taking Blueprint for Wellness and will be eligible for up to 120 Wellness Points based on healthy or improved results. Participants/spouses must be enrolled in the Virgin Pulse activity program to earn HealthCash.
Any eligible participant who cannot access a Quest Diagnostics facility or on-site screening may be eligible for the HealthCash and Wellness Points incentives if his or her personal physician completes and faxes the Physician Results Report Form by the July 31 deadline.
Please use Blueprint for Wellness Communications Toolkit materials (February 2015 toolkit) to further encourage participation and preregistration. You can find the Toolkit and Physician Results Report Form on the General Board extranet (username: extranet; password: gbop!123); browse under “HealthFlex Tools & Resources.” (Note Quest Diagnostics’ new phone number for preregistration: 1-855-632-9355.)
