We Want to Know if You Plan to Host a Blueprint for Wellness On-site Event

To best support you, Wespath will soon send a survey to plan sponsors to learn if you intend to have an on-site event for your participants and covered spouses to complete the Quest Blueprint for Wellness®. Last year, most plan sponsors decided not to have on-site events due to COVID-19 and the cancellation of most in-person gatherings.

If desired, Quest Diagnostics® can support on-site events with COVID-19 safety protocols in place. Events should have at least 20 people to avoid additional fees. Each appointment will consist of a blood draw and a few measurements.

Most people covered by HealthFlex have four options to complete the Blueprint for Wellness by August 31 and earn the $100 Pulse Cash incentive: an on-site event, an appointment at a local Quest lab, a Self-Collection Kit (previously called a Qcard) and a physician form. The Self-Collection Kit is available to everyone except residents of New York.
