Special Notice: Important Reporting and Form Due Dates

We know it's a busy time of year for reporting, completing adoption agreements, and more. Please review the following items that are either past due or have due dates right around the corner. If you've completed and submitted all items below, thank you! If you haven't had a chance to do so, please complete and submit any outstanding items as soon as possible.

• Past Service Rates (PSR) Adoption Agreements – Due June 30. Please review the June 2 On Board Express for more information.
• Actuarial End of Year Surplus Intention Forms – Due June 30. Please see May 31, 2022 email from Tyler.
• List of Disaffiliated/Closed Churches – Due July 8. Please return the completed Excel file to your Client Service Manager.
• Clergy Appointment Changes – Due July 7 for July billing. If you have clergy who have withdrawn under paragraph 360, please enter those as soon as possible for account conversion and so we can move the individual to a new plan sponsor, if needed.
• Comprehensive Benefit Funding Plan – Due July 29.

Some conferences experienced issues adding paragraph 346.1 appointments. Please send the appointment information to your Client Service Manager if you experience this issue. We are working to have this corrected as soon as possible.

For HealthFlex or Via Benefit Sponsors:

• HealthFlex Adoption Agreement – Due June 30 for 2023 elections. Please return Exhibit B (Exhibit A if you are making eligibility changes).
• Via Benefits Adoption Agreement – Due July 8 per June 16 email. This is a new format from prior years.
